- Freestone frame, strapwork flanking and straight entablature; above, an achievement and flanking part-scrolls; M.I. (shallow-engraving, gilded) on black tablet, set portrait-wise; decorated apron with more part-scrolls.
- Anne Heath, 16 May 1602 - 6 Sep 1602; daughter of Sir Robert Heath, lawyer, and his wife Margaret.
